Eligibility Name : This will be automatically fetched depending on the EligibilityID type and EligibilityID submitted above. This field will be displayed only for the following EligibilityID Type values.

In case of the remaining EligibilityID Type values, you need to enter the proprietor's name in this field. Note that the proprietor should be an individual.
